The girl froze to death on the steps of her apartment complex. Her cousin, Rachel Downer, was charged in connection with the death.
According to a CHIPS petition, Downer was offered family assessment, but declined, from October 2013 to January 2014 following allegations that her child was in an apartment with no adult supervision with a 6-year old and 3-year old. From August to October 2012 she was offered services for her and her son, but the case was closed after she did not cooperate with a social worker. In August 2012, she was offered voluntary outreach services, but declined.

In a June 2013 report the county was told that the boy was being abused by Warr, and despite the active restraining order the mother continued to let Warr stay at the house. A family assessment worker visited the home on June 7, 2013, three days before the child died.
The boy was in the car with his mother, who was drunk and got into an accident. Unrestrained by a seatbelt, the 6-year old was ejected from the vehicle and found 30 yards away.

Dontrell died of apparent sudden infant death syndrome after he was found face down on a mattress while sleeping with other children.
The boy's mother, Freda Purdue, was twice found to be drinking heavily while caring for her children and falling asleep on the boy while under the influence of alcohol. Family assessment was offered following a September 2011 investigation. The children were removed from the custody of their mother, Deqa Yusuf, in January 2012 due to her drinking while caring for children. Both parents were found responsible for maltreatment. They were returned on May 10, 2012. On May 31, 2012, the girl was found drowned.
Cottrell was beaten to death and suffered from a blood infection after a burn on his hand went untreated. His mother, Shacara Foster, and her boyfriend, Kentae Todd, were convicted for the death.
There are multiple child protection reports pertaining to adults living at the home where Cottrell died. The mother had previously abandoned her child, and was found responsible for neglect after another child died of sudden infant death syndrome. In February 2008 drugs were found in the home following a raid and the children were put into emergency protective custody. In 2008, Kasyiah Todd (the mother of man convicted of killing Cottrell) was offered family assessment after a report that her children were being physically abused. In November 2010 the county again offered Todd's family family assessment after a report that their home was unsafe and unsanitary. Kentae Todd had been found responsible for breaking his daughter's ribs in 2010. Four days before Cottrell Short was killed, a neglect report was made to the county pertaining to Cottrell. The county screened out the report, and a few days later offered voluntary services to the parent. That day, Cottrell died.

An abuse report made and screened in on Aug. 8, a month before Ariel's death. Norman County Social Services said the case was opened due to the mom not supervising the children. The case was screened in for family assessment and a case was open when the girl died.
Byron died in a car accident after his mother, Cindy Rewertz, knew that the driver was drunk. The driver died. His mother was found responsible for neglect.
According to the CHIPS petition, child protection was involved with the family since 2001. There were multiple reports of drug use by the parents, and prior sex assault claims. According to St. Louis County, a report was made in October 2010 alleging that the boy's mother was neglecting another child. In March 2011 the county received another report that the boy's mother was abusing drugs and alcohol. That case was closed in August 2011. The next month the county received another report that Rewertz's abuse of alcohol and drugs impacted her ability to parent Byron. That case was referred to family assessment and was open when Bryon died in December 2011.

Quentin's father was drunk when he crashed his car, killing his 2-year old son, who was not properly restrained in his car seat.

Adrian's mother, Toni Medrano, was drunk on Nov. 21, 2011, when she fell asleep on the couch with her baby, suffocating him. She had been drinking steadily since the baby was born.
In 2006, Medrano was referred to family assessment following allegations of child neglect. In July 2009, she was again referred to family assessment after continuing concerns about the children being unsupervised. She was again referred to family assessment in January 2010 after five of the children, ages 1 to 9, were home alone. In March 2011, Medrano was reported for educational neglect. That case was closed in July 2011.
The father, Willie Franklin Sr., placed his crying son facedown on a pillow. He then smoked marijuana and called a sex chat line as the child suffocated.
A CHIPS petition was filed against the boy's mother, Susan Phang, in 2007, after she was accused of neglecting and abandoning her children. According to the petition, Phang was accused of selling crack with Willie Franklin from their home and using the drug in October 2007. Also according to the petition, there was a screened out report that month for child neglect against Phang. In 2005, Olmsted County responded to a report that Phang was assaulted by another man in front of her children. That case was referred to family assessment, but no ongoing services were provided. According to Hennepin County, child protection received reports in October 2007 and November 2007 that were screened out.
Deanna's caregiver purchased alcohol for the girl, which she drank. The caregiver then had Deanna drive, as the caregiver didn't have a valid license. Deanna pulled into traffic, and was hit by another vehicle and pronounced dead at the scene.
A child protection report was received in November 2007 due to a report that the girl was in detox as a result of drinking with parents. Report findings "indicated that child reported that she had been drinking and other adults were there but she was not drinking with her parents and her parents did not supply the alcohol." The county was unable to substantiate child maltreatment.
Isaiah's death was called a homicide by the medical examiner with multiple reasons listed on the death record for causes, including respiratory arrest, severe sleep disorder, and cerebal palsy with an injury listed as "obstructive sleep apnea, was not wearing cpap at time of death." Alexander's brother was placed in long-term foster care in 2007 and was reunited with his mother that year. In 2007, the mother was found responsible for maltreatment when Alexander's brother was shaken. In 2009, the county received two reports about his brother's behavior. A case was opened and ongoing when Alexander died.
In either 2002 or 2003, Dakota County opened a case against Jesse Hummel after getting a report that his 3-month old baby had been abused. The boy had bleeding on his brain that required surgery and injuries to his head and face including bruising on his jaw line. Hummel pleaded guilty to felony malicious punishment of a child.
The girl had a skull fracture. Her father, Lance Ballinger, pleaded guilty to assault in the death. Mille Lacs child protection said he was intoxicated when caring for Tila.
Mille Lacs child protection completed one previous child maltreatment investigation from a report received on Nov. 5, 2009 due to allegations that the girl was physically abused from a head injury. The girl had partial skull fractures. Child protection was told that an older sibling picked Tila up and dropped her on the head. A consulting physician report indicated that the information reported was quite possibly consistent with injury. An investigation was switched to family assessment due to the cooperation of parents.
Samantha was raped and murdered by her uncle
According to Nobles County, the agency received a report in June 2007 that was accepted as a family assessment. According to a CHIPS petition, the report pertained to the girl's father doing drugs in the home. Children were placed in foster care. In January 2008, the agency received a child welfare report. No investigation was conducted.
Jessica died of malnourishment, dehydration, delayed growth and bilateral bacterial pneumonia. Her mother, Melissa Kunkel, was charged with child neglect.
According to a court petition, the child's mother "had a significant child protection history in multiple states" and "moved around" to avoid social services. In October 2008, Olmsted County received a report that her 6-year old child had missed multiple days of school. The case was closed in December after a county social worker was only able to meet with the mother once. The girl died that month.

When the baby was born there was suspicion that she was positive for THC. A lab test determined that the mother was not positive for amphetamines, and the positive opiate results were a result of the medications administered at the hospital.

The boy was sleeping with his mother. When she awoke, he was found lying facedown inside the bedroom closet with a plastic bag stuck to his face. She told police that the night before she took prescription medication that "reportedly" causes her "to sleep hard."
In 2008, Milwaukee County received a reported that the mother exposed her newborn son to controlled substances while she was pregnant. She tested positive for cocaine when she gave birth to the boy. According to the CHIPS petition, when she and her husband moved to Minnesota they had not completed services.

Nevaeh's father, Jamie Andrews, was convicted of manslaughter after being accused of shaking the baby to death.
Five months prior to Nevaeh's death, a report was made to Hennepin County in January 2007. Maltreatment was determined, but services were deemed not needed. According to the juvenile court report, the child's mother said Andrews kicked down the door of her apartment, demanded to know where her gun was, and started beating her. One of the punches hit Nevaeh. The mother reported to Hennepin Child Protection that she had a history of domestic incidents with Jamie Andrews.
Cecilia's mother, Katherine Bodem, drowned her baby in a bathtub.
The infant was assaulted by his 30-year old babysitter.

The girl was removed from her parents custody two weeks before her death after she was found to have a complex skull fracture. The child was put into emergency foster care. The parents denied the abuse, but child neglect was determined because the father did not seek medical services for more than two days after the incident. The girl was returned to the home on Jan. 31.
The girl was violently shaken by her mother's boyfriend, Ahmed Mohamed. Her death was ruled a homicide due to massive internal injuries to the chest and abdomen, contusions to the chest and back, severe liver laceration, and internal bleeding. Mohamed pleaded guilty to second-degree unintentional murder. Sahra's mother, Khadijo Ali, pleaded guilty to 2nd degree manslaughter.
Ten reports were made regarding the family dating back to 2003 prior to Sahra's death. Among those reports: In 2006, Hennepin County was told Sahra's mother was using drugs and not caring for her children. The next month, the county received a report that Ali abandoned two of her children.
Jordan died after being placed in a scalding hot bathtub and not getting medical attention afterward. Seventy-three percent of her body was scalded. Her father, Jason Gonsioroski, was convicted of second degree murder. His girlfriend, Julie Ann Meier, was also convicted of second degree murder.

Malaya's father, Pierre Gray, beat the girl to death after a toilet-training accident. Both Gray and Douglas' stepmother, Brandilyn Douglas, were found responsible for maltreatment.
In 2002, Brandilyn Douglas lost custody of another child after she was accused of abusing drugs and one of her children tested positive for cocaine; in 2004, Malaya Douglas was placed in foster care as a baby when she and her mother tested positive for drugs at birth in 2004. In 2005, a family assessment was conducted following a report that drugs and guns were found in the home of Malaya's father and stepmother.

Demond was found in the closet in home of aunt, Carla Poole. An autopsy showed he had a variety of bites, cuts and bruises over his body, and that he was in a great deal of pain before he died. Poole admitted to hitting the child and hiding his body. She told police she beat him as a result of a toileting accident.
There were six reports made about Poole prior to Demond's death about her own children, including a 2005 report that she neglected her kids and lived with a man who sold drugs out of the home. The next year she was named as a suspect in a severe burning incident of a child who had been in her care, but there was insufficient evidence to indicate Poole was the perpetrator.
Imani was found face down in a laundry basket where she regularly slept; there was no crib found in the home.
